The U.S. Department of State has added conditions such as obesity, cancer, heart disease, and diabetes to the list of reasons for which visa issuance can be denied. The new guideline was sent to all diplomatic missions in November 2025.

According to The Washington Post, the Department of State reminded consular staff that an applicant's health is an important factor in visa matters. The document noted that some conditions—cardiovascular diseases, respiratory diseases, cancer, diabetes, and mental health-related issues—could require treatment costing "tens of thousands of dollars."

The guideline also mentioned obesity as a factor that can increase the likelihood of denial. The reason is that it can lead to problems such as asthma, sleep apnea, high blood pressure, and depression.

The new manual also requires checking whether the applicant has sufficient funds for treatment in the U.S., as well as assessing the likelihood of them needing public assistance. According to KFF Health News, such criteria are aimed at preventing the applicant from becoming a "public charge."

Some sources at the Department of State informed The Washington Post that the document was prepared by political leadership and did not undergo the usual expert review.

White House representatives, however, emphasized that the new approach is based on a 100-year practice and is aimed at ensuring no excessive financial burden falls on taxpayers.
